Volunteer Liability Waiver
Please read the below information and check the box at the bottom to agree to the Liability Waiver and Photo Release.
I acknowledge and understand that volunteering with animals may pose significant risks, including the potential for serious injury or even death. By participating in volunteer activities at the Clay County Animal Shelter, Inc. (CCAS), I voluntarily assume full responsibility for any risks and liabilities associated with my involvement.
I agree to INDEMNIFY and HOLD HARMLESS CCAS, its officers, employees, agents, and representatives from any and all claims, liabilities, losses, costs, damages, attorney’s fees, or expenses arising out of my volunteer activities, whether these occur on-site or off-site, or from any failure to comply with CCAS's policies, rules, or procedures.
I agree CCAS is not liable for any damages resulting from the actions of third parties or animals. I further agree to release and hold harmless CCAS from any claims or damages arising from my own negligent actions or omissions, including any accidents, injuries, or property damage that may occur during my volunteer service.
I understand and accept that, as a volunteer, I am not considered an employee of CCAS and am not entitled to any form of compensation or benefits, unless otherwise required by law.
I agree to follow all CCAS protocols and procedures without objection including, but are not limited to, refraining from entering unauthorized areas, not handling animals located in restricted areas due to health/safety risks, not bringing cameras onto the premises without explicit permission, not misrepresenting myself as an employee of the Shelter, and ensuring all gates and doors remain securely locked at all times.
Photo Release
I understand that during my time as a CCAS volunteer photographs and/or videos of me may be taken at various events or during projects. I authorize CCAS to use my image in any publications, including online, without compensation or claim on my part, now or in the future.
